Just did the form online. If you are travelling by vehicle from Canada with your dog (s), it is very simple. They seem more interested in you than the dog, The form changes as you fill it out with sections added or deleted. By land, No destinations needed, no proof of vaccination or micro chips. Just the dogs name , breed , age in years and months, gender and description. They email you a copy and it is good for multiple entries and expires every 6 months. That's it.

Re: Dog importation to the US

I live right on the border and cross multiple times a week. I have been invited to hunt with US CBP officers on the USA side, and will likely host a couple of them on our side this season. I cross with my bird dogs regularly. This is not official, and may only apply at smaller crossings, but direct from the CBP officers: “we have been given zero information on how or what to enforce, therefore, we will not be enforcing anything until they actually provide guidelines”. Given the new ease of doing the lesser online form, it’s best to do it, but don’t be surprised if they don’t ask anything about your dog.
If we’re not supposed to eat animals, how come they’re made out of meat?
