It happens. I have a few pics of guys with arrow wounds. Rock Doctor is right, check your arrows. It can also happen if your arrow falls off the rest just before release, or if it breaks upon release.

This pic has been circulating for sometime. I'm sure there is a story, but I suspect the picture tells it well enough.
